This is a combination, and each component handles a different part of the problem. Cetirizine blocks the H1 histamine receptor. Histamine released from mast cells is what produces the sneezing, itching, watery eyes and runny nose of an allergic reaction, and this newer generation of antihistamine blocks it while barely entering the brain, so the drowsiness of the older agents is largely avoided. Phenylephrine narrows the swollen blood vessels in the lining of the nose. It is that swelling, not mucus alone, that blocks the nose during a cold, so shrinking the lining opens the airway and relieves the stuffiness and sinus pressure. Dextromethorphan acts to suppress cough. The target is a dry, unproductive cough, where coughing serves no purpose and only irritates the throat into more coughing. It works by raising the threshold of the cough centre in the brainstem, so minor throat irritation no longer sets the reflex off. Sodium citrate acts as an expectorant, making airway secretions less viscous and easier to bring up. Paracetamol reduces fever by acting on the temperature-regulating centre in the hypothalamus, which resets the body's thermostat back to normal, and raises the pain threshold centrally. It has very little effect on the stomach lining, which is why it is the first choice for fever and everyday pain in children and adults alike. Combining them means one preparation covers what would otherwise need two or three.
