This is a fixed-dose combination, and each component works on a different mechanism. Telmisartan blocks the angiotensin II receptor. Angiotensin II is the body's most powerful natural vasoconstrictor: it tightens arteries, drives salt and water retention through aldosterone, and over years thickens the heart and stiffens the kidney's filters. Blocking its receptor lowers pressure and, separately from that, protects the heart and kidney from the remodelling it causes. Telmisartan has the longest half-life in the group, around twenty-four hours, which is why it still holds pressure down in the early morning hours when cardiovascular events cluster. Cilnidipine blocks L-type calcium channels in the muscle wall of the arteries. Calcium entry is what makes that muscle contract, so blocking it lets the artery relax and widen. Wider arteries mean lower resistance for the heart to pump against, which brings blood pressure down and reduces the work the heart muscle has to do. Cilnidipine differs from the others in also blocking N-type channels on sympathetic nerve endings. That is why it tends to cause less ankle swelling and less reflex quickening of the pulse than amlodipine, and why it is often chosen when a patient cannot tolerate those effects. Metoprolol blocks beta-1 receptors on the heart. Adrenaline acting on those receptors is what drives the heart to beat faster and harder, so blocking them slows the rate, softens the force of contraction and lowers the oxygen the heart muscle needs. In angina that reduced demand is the whole point; in hypertension it is the reduced output that lowers pressure. The extended-release form holds a steady level through the day, which is what the heart failure evidence for metoprolol succinate is based on. Bringing them into one tablet means fewer tablets a day, which in a lifelong medicine is one of the strongest predictors of whether it actually gets taken.
