Permethrin holds open the sodium channels in the nerve cells of the scabies mite and of lice, so the nerve fires continuously until the parasite is paralysed and dies. Mammalian nerve cells break permethrin down rapidly and are barely affected, which is why it is the first-line scabies treatment including in young children and in pregnancy.
