How Mervin-SB 1.5 works

This is a combination, and each component has a distinct role. Meropenem binds a wider range of the penicillin-binding proteins than any other beta-lactam and resists almost all beta-lactamases, which gives it the broadest antibacterial spectrum available in a single agent. That reach is exactly why it is held in reserve: it is used for severe or resistant infection, and using it casually is what drives carbapenem resistance. Sulbactam has little antibacterial action of its own. Its job is to be attacked. Resistant bacteria defend themselves by producing beta-lactamase enzymes that break the antibiotic's beta-lactam ring; the inhibitor binds those enzymes and occupies them, so the antibiotic passes through intact. That is why the combination works where the antibiotic alone no longer does. Supplying them in one vial means the pair is reconstituted and given together, in the ratio intended.
