This is a fixed-dose combination, and each component works on a different mechanism. Torsemide blocks the sodium-potassium-chloride transporter in the loop of Henle, the segment of the kidney that reclaims most of the filtered sodium. Blocking it there produces a much larger fluid loss than a thiazide can. Torsemide is absorbed more predictably than furosemide and lasts longer, which matters in heart failure where inconsistent absorption causes swings in fluid balance. Spironolactone blocks the receptor that aldosterone acts on in the kidney. That clears sodium and water while holding on to potassium, unlike other diuretics. More importantly in heart failure, aldosterone also scars and stiffens heart muscle over time, and blocking the receptor interrupts that remodelling. Bringing them into one tablet means fewer tablets a day, which in a lifelong medicine is one of the strongest predictors of whether it actually gets taken.
